It’s an everyday thing for pharmacist to advise customers on their medications, but some are not heeding the advise.
The Federal Government's Institute of Medicine estimates there are 1.5 million adverse reactions to prescription drugs each year in the U.S.
Around 75,000 occur when people take a dangerous combination of drugs.
Pharmacist Ira Katz says, that "A lot of times they're confused. They are taking two, three, multiple medications and they just don't have a good handle on it."
Mixing the wrong drugs can sometimes reduce their potency or effectiveness and in extreme cases can be fatal.
Experts recommend keeping a list of all drugs you are taking including vitamins and herbal supplements. Read the paperwork that comes with your prescription. And talking with a pharmacist also can help prevent mistakes.
