An investigation into an accident that killed a coal truck driver this summer shows that alcohol played a factor.

An investigation into an accident that killed a coal truck driver this summer shows that alcohol played a factor.

Mark Allen Gray drowned when the truck he was driving rolled over into a sediment pond at Catenary Coal's Samples mine July 28.

The 28 year old was working for contractor Hawkeye Contracting.

Thursday's report by the Office of Miners' Health Safety and Training says a medical examiner's review showed that Gray had a blood alcohol level of 0.08 percent. The report also reported traces of marijuana in his blood.

Hawkeye Contracting was cited for failure to comply with workplace safety regulations. Foreman Phillip Rife and Appalachian Security also were cited for failing to comply with regulations designed to keep alcohol off mine sites.
